MMS Express CD Manufacturing Order Form
Visit the Instant Online Quote page to complete this form, which helps your rep prepare your order. This form is not absolutely necessary, but it can help streamline your order.

Graphics Form
Your graphic designer should complete this form, then you include it with your artwork disc, or upload it to the MUSIC MANUFACTURING SERVICES design department using EASI along with the artwork. Download Graphics Form


Mastering Information Form
You will need this form if you take advantage of MMS' superior mastering services. Simply download the form, complete, and include with your source files for mastering. Click here for more information on MMS mastering services.

Intellectual Property Rights Form (IPR)
This form provides proof of copyright ownership and helps prevent unlawful reproduction of music and other copyrighted materials, including yours. Your CD order will NOT BE PROCESSED without an IPR form. (See Before You Order for more information.)
